This is an IVORY with IVORY lace blouse made by Frontier Classics. (I also have white in my eBay store).
It has a stand up collar that stands up just enough to look great, but not enough to be uncomfortable!
The sleeves are gathered and puffy at the top and the end of them is gathered into a cuff with 4 working buttons.
The blouse has working  buttons down the front and is trimmed with lace on the front of the blouse, and at the cuffs.
Manufacturers can vary greatly,
so I like to go by measurements
. The measurements below are the actual measurements around the bust, so you will need to add 2-4" for comfort and ease.  OR you can measure a shirt that you like the way it fits and buy one of these shirts that measure the same.
This shirt would look fantastic with an old west skirt and boots, both that I have in my eBay store!
